目的：慢性特発性血小板減少性紫斑病（慢性ITP）患者を対象に，使用実態下でエルトロンボパグを長期投与した際の安全性および有効性を評価することを目的とした使用成績調査を実施した。
方法：本調査の実施期間は2010年から2020年であった。観察期間はエルトロンボパグ投与開始後1年間とし，継続調査が可能な症例は最長2年間とした。
結果：安全性解析対象症例2019例において，女性割合が59.88％，65歳以上の高齢者割合が51.46％であった。497例に749件の副作用が認められ，副作用発現割合は24.62％であった。1.00％以上発現した副作用は，肝機能異常，頭痛，肝障害，血小板数増加，深部静脈血栓症，倦怠感，脳梗塞であった。重点調査項目に設定した血栓塞栓症の副作用発現割合は4.36％であった。有効性解析対象症例2018例において，血小板数，出血傾向，QOL，全般改善度の4項目を評価した結果，投与継続できた症例においてエルトロンボパグ投与中の長期的な有効性が認められ，血小板数および出血傾向改善割合は投与中止・終了後4週まで維持された。安全性および有効性に影響を及ぼす要因について検討したが，新たに注意を要する要因は認められなかった。
結論：使用実態下で慢性ITP患者に対してエルトロンボパグを長期投与した際の安全性および有効性について，承認時の臨床試験（国内第Ⅱ/Ⅲ相試験）との大きな乖離や，新たに注目すべき傾向はみられず，特段対処すべき問題点は認められなかった。

Objectives: A drug use-results survey was conducted in patients with chronic idiopathic thrombocytopenic purpura (ITP) to investigate the safety and effectiveness of long-term administration of eltrombopag in the real-world setting.
Methods: The period of this survey was from 2010 to 2020. The observation period was 1 year after the start of eltrombopag administration, and was extended up to 2 years in patients who could participate in the survey for more than 1 year.
Results: The safety analysis set (SAF) included 2019 patients, female patients accounted for 59.88%, and patients aged 65 or older accounted for 51.46%. In the SAF, 749 adverse drug reactions (ADRs) occurred in 497 patients, and the incidence of ADRs was 24.62%; the ADRs reported in ≥ 1.00% of patients were hepatic function abnormality, headache, liver disorder, platelet count increased, deep vein thrombosis, malaise, and cerebral infarction. The incidence of thromboembolism, which was set as a priority survey item, was 4.36% . The effectiveness analysis set included 2018 patients, and the four items of platelet count, frequency of bleeding episode, QOL, and overall improvement were evaluated. Long-term effectiveness of eltrombopag was observed in patients who could continue the eltrombopag administration, and the improvements in platelet count and frequency of bleeding episode were maintained up to 4 weeks after discontinuation or termination of eltrombopag. Factors affecting the safety and effectiveness of eltrombopag were examined, however no new factors of concern were identified.
Conclusions: In this survey, the safety and effectiveness of long-term administration of eltrombopag were investigated in patients with chronic ITP in the real-world setting, and there were no significant differences in the results compared with the previous clinical trials (phase II/III study conducted in Japan). Furthermore, no new safety factors were observed. Therefore, no new concerns regarding eltrombopag were identified in this survey.

大麻は古来より疾患の治療に用いられていたが，長い間その薬理作用は不明であった。しかし，内因性カンナビノイドとカンナビノイド受容体の発見により1990年代以降に徐々にそれが解明されてきた。
大麻草にはカンナビノイドと呼ばれる薬理作用を持つ成分が120種類以上含まれており，その中心となっているのがΔ-9テトラヒドロカンナビノール（以下THC）とカンナビジオール（以下CBD）である。THCは向精神作用から規制の対象となっているが，CBDは向精神作用を持たず，乱用の危険性もない。その安全性と臨床効果が近年注目され，急速に研究が進んでいる。CBD製品は海外では医薬品として難治性のてんかんに使用されているが，日本ではCBDに対する医師の認知度が低く，医療で使用される機会はほとんどない。
本論文では，医療用途の大麻の歴史，CBDの薬理学とその臨床応用について述べる。

Cannabis plants and resins have been used for therapeutic purposes since ancient times, but those pharmacological actions have long been unknown. However, the discovery of endocannabinoids and those receptors has been gradually elucidated since the 1990s.
The cannabis sativa containing more than 120 cannabinoids has pharmacological actions, the main of which are Δ-9 tetrahydrocannabinol (THC) and cannabidiol (CBD). THC is regulated for its psychotropic effects in almost all countries, but CBD does not have psychotropic effects and no risk of abuse in recent research. So, the safety and therapeutic potential of CBD have been attracting attention. The CBD products are used as medicines for refractory epilepsy in other countries, still there are a few chances for medical use in Japan because the awareness of CBD among physicians is very low.
This review will focus the history of cannabis for medical use, the pharmacology and therapeutic potential of CBD.

最近、マスコミやSNSで「CBD」という言葉をよく見かけるようになり、日本でも特に若い世代を中心に関心が高まっているように感じます。また、医療分野でも、小児の難治性てんかんに対するCBD医薬品の臨床試験が始まり、やっと大きな一歩が踏み出された感があります。
わが国でも多くのCBD製品が販売されるようになりましたが、製品の説明書を読んでも、「CBDがどのように効くのか」についてきちんと書かれているものは、ほぼありません。購入後に不信・不安を抱く方も多いと思いますが、実は、「作用メカニズムがよく分かっていない」とうのが正解です。ですから、説明しようにも上手く説明できないのです。私は「カンナビジオール（CBD）の薬理学と臨床応用」という論文を書かせていただき、執筆にあたりCBDに関するあらゆる文献を調べました（その数3,000本以上！）が、メカニズムが複雑すぎるのと、まだはっきり解明されていない部分が多すぎて、「たぶんこういうふうに作用している…であろう」というあいまいな書き方しかできませんでした。
こうした背景から、一般の方がCBDを十分理解することは難しい（不可能）のですが、私も、患者さんにCBDを使用している立場として、「患者さんが納得して使用できるよう」さまざまな質問に対する答えをいつも考えています。「患者さんから頂くよくある質問に私がどう答えるのか？」これが本稿のテーマです。多くの方が抱いているCBDに関する疑問とその答えを、できるだけ分かりやすく、対話形式で書いていきたいと思います。
