ジェネリック医薬品のペランパネル錠4 mg「サワイ」と先発医薬品であるフィコンパⓇ錠4 mgとの生物学的同等性を検証するため,日本人健康成人男性を対象に2 剤2 期のクロスオーバー法による生物学的同等性試験を行った。
治験薬投与後,最終採血時点までの血漿中ペランパネル濃度を測定し,有効成分の血漿中濃度-時間曲線下面積(AUCt)と最高血漿中濃度(Cmax)を指標として,両製剤のバイオアベイラビリティを比較した。その結果,両製剤のAUCtとCmaxの対数値の平均値の差の90%信頼区間は,生物学的同等性の判定基準を満たした。
本治験において認められた有害事象はいずれも軽度で回復または軽快が確認されており,被験者の安全性に問題はなかった。
以上の結果より,ペランパネル錠4 mg「サワイ」は,フィコンパⓇ錠4 mgとの生物学的同等性が検証された。

ジェネリック医薬品のシタグリプチン錠50 mg / 100 mg「サワイ」と先発医薬品であるジャヌビアⓇ錠50 mg / 100 mgとの生物学的同等性を検証するため,日本人健康成人男性を対象に2剤2期のクロスオーバー法による生物学的同等性試験を行った。
治験薬投与後48時間までの血漿中シタグリプチン濃度を測定し,有効成分の血漿中濃度-時間曲線下面積(AUCt)と最高血漿中濃度(Cmax)を指標として,両製剤のバイオアベイラビリティを比較した。その結果,いずれの試験においても,両製剤のAUCtとCmaxの対数値の平均値の差の90%信頼区間は,生物学的同等性の判定基準を満たした。
いずれの試験においても,有害事象は認められなかった。
以上の結果より,シタグリプチン錠50 mg / 100 mg「サワイ」は,ジャヌビアⓇ錠50 mg / 100 mgとの生物学的同等性が検証された。

Background: Patients with non-muscle-invasive bladder cancer (NMIBC) and muscle-invasive bladder cancer (MIBC) in Japan face trade-offs among oncologic control, toxicity, and lifestyle impact that can trigger decisional conflict. We aimed to quantify health-related quality of life (HRQoL) and decisional conflict across standard of care (SOC) to identify unmet needs.
Methods: We conducted a cross-sectional survey of adults with NMIBC or MIBC treated within the previous 6 months in Japan across four cohorts: Bacillus Calmette-Guérin (BCG)-treated NMIBC without radical cystectomy (RC); BCG-unresponsive NMIBC with RC; RC-treated MIBC; and bladder-preserved MIBC. HRQoL was assessed using EORTC QLQ-C30 and EORTC bladder cancer (BC) module; decisional conflict with Decisional Conflict Scale (DCS).
Results: We analyzed 215 patients (174 males, 41 females; age 74.9 ± 7.8 years). Global HRQoL was similar across cohorts (mean range 63.6–72.9). All cohorts reported at least moderate decisional conflict (higher with poorer HRQoL) and some functional impairments, with future worries a high BC-specific concern (range 23.6–33.9). Specific impairments varied: the BCG-treated NMIBC without RC cohort scores did not meet clinical relevance thresholds. In contrast, the BCG-unresponsive NMIBC with RC cohort suggested clinically relevant
physical/cognitive functioning impairments and high symptom burden. The RC-treated MIBC cohort had clinically relevant physical functioning impairments, dyspnea and nausea/vomiting. The bladder-preserved MIBC cohort suggested similar symptom and functional impairment.
Conclusions: BC patients receiving SOC in Japan experience therapy-specific HRQoL impairments and
decisional conflict. Addressing these unmet needs requires individualized treatment strategies, including tolerable, bladder-preserving therapies and enhanced shared decision-making support.

LDL-C低下療法により心血管イベントリスクは低下するが,LDL-C以外の残余リスクとして高TG血症が注目されている。ペマフィブラートは従来のフィブラート系薬剤と比較して安全性に優れた高TG血症治療薬として承認され,2023年には1日1回投与の徐放性(XR)製剤が使用可能となった。本研究では,高TG血症を有する脂質異常症患者59例を対象に,ペマフィブラートXR錠0.2 mgから0.4 mgへの増量前後の臨床検査値を比較した。増量後,血清TG値は273 mg/dLから162 mg/dLへ有意に低下した(P<0.001)。また,ALTは28 U/Lから25 U/Lへ(P=0.002),ALPは74 U/Lから62 U/Lへ(P<0.001)有意に低下し,肝胆道系指標への好影響も示唆された。一方,TC,HDL-C,LDL-C,ASTおよびCreには有意な変化を認めなかった。さらに,増量に伴う重篤な有害事象や投与中止例は認めなかった。以上より,ペマフィブラートXR錠0.4 mgへの増量は,高TG血症に対する有効かつ安全な治療戦略であり,実臨床におけるXR 錠0.4 mg 増量の有効性および安全性を支持する結果と考えられた。

Clinical Efficacy and Safety of Dose Escalation from 0.2 mg to 0.4 mg of Pemafibrate XR in Patients with Hypertriglyceridemia

Lowering low-density lipoprotein cholesterol (LDL-C) reduces the risk of cardiovascular events; however, hypertriglyceridemia has recently gained attention as an important residual cardiovascular risk factor beyond LDL-C. Pemafibrate, a selective peroxisome proliferator-activated receptor alpha modulator, has been approved as a treatment for hypertriglyceridemia with a more favorable safety profile than conventional fibrates, and an extended-release (XR) formulation allowing once-daily administration became available in 2023.
In this study, we retrospectively evaluated changes in clinical laboratory parameters in 59 patients with dyslipidemia and hypertriglyceridemia who underwent dose escalation of pemafibrate XR from 0.2 mg to 0.4 mg daily. Following dose escalation, serum triglyceride (TG) levels significantly decreased from 273 mg/dL to 162 mg/dL (P < 0.001). In addition, alanine aminotransferase (ALT) levels significantly decreased from 28 U/L to 25 U/L (P = 0.002), and alkaline phosphatase (ALP) levels decreased from 74 U/L to 62 U/L (P < 0.001), suggesting favorable effects on hepatobiliary parameters. In contrast, no significant changes were observed in total cholesterol (TC), high-density lipoprotein cholesterol (HDL-C), low-density lipoprotein cholesterol (LDL-C), aspartate aminotransferase (AST), or serum creatinine (Cre). Furthermore, no serious adverse events or treatment discontinuations associated with dose escalation were observed.
These findings suggest that dose escalation of pemafibrate XR to 0.4 mg is an effective and well-tolerated therapeutic strategy for patients with hypertriglyceridemia, providing real-world evidence for its clinical utility.

目的:当院(すずひろクリニック)におけるペマフィブラート投与患者の特性,治療効果,安全性を実臨床下で検討することを目的とした。
方法:2018年6月~2025年6月に当院でペマフィブラートを投与した123例を後ろ向きに調査した。
結果:投与した症例は男性70名,女性53名であった。ペマフィブラートとして0.2 mg/日を投与した症例の随時TG値は,366.9±161.9 mg/dLから232.2±132.2 mg/dLにまで有意に低下した(p<0.001,n=74)。175 mg/dL未満に到達したのは43.2%だった。ペマフィブラートとして0.4 mg/ 日を投与した症例の随時TGは,376.5±187 mg/dLから190.7±147.1 mg/dLまで有意に低下した(p<0.001,n=75)。175 mg/dL未満に到達したのは64%だった。副作用による中止は0例だった。
結論:ペマフィブラートは実臨床において有効かつ安全に使用できる薬剤である。

目的:トレプロスト吸入療法を受けている肺動脈性肺高血圧症患者および間質性肺疾患に伴う肺高血圧症患者を対象に,本剤治療に対する総合満足度,息切れ症状および日常生活動作の変化を患者報告アウトカムの観点から評価し,実臨床における患者視点の治療実態を明らかにすることを目的とした。
方法:本研究はWebアンケート調査による観察研究であり,アンケート回答時点でトレプロスト吸入療法開始後30日以上が経過している患者を対象として実施した。治療に対する総合満足度,息切れ症状および日常生活動作の変化に加え,吸入操作やネブライザ・薬剤管理の負担,外出時の治療実施状況等について解析した。
結果:276人から有効回答を得た。59.1%の患者が本剤治療に満足していると回答し,53.3%が息切れ症状の改善を感じていた。日常生活動作では変化なしと回答した割合が59.4%と最も多かった。91.7%の患者が自己管理可能であり,54.7%の患者が外出先においても吸入療法を実施していた。
結論:トレプロスト吸入療法は良好な総合満足度および息切れ症状の改善が得られるとともに,多くの患者が吸入操作およびネブライザ・薬剤管理を自立して実施可能な治療法であることが患者報告アウトカムを通じて示された。

Real-World Treatment Experience with Treprost Inhalation: Patient-Reported Outcomes in Patients with PAH and PH-ILD

Objective: To evaluate patient-reported outcomes, including overall treatment satisfaction, changes in dyspnea symptoms, and activities of daily living, in patients with pulmonary arterial hypertension and pulmonary hypertension associated with interstitial lung disease receiving Treprost Inhalation, and to characterize the real-world treatment experience from the patient perspective.
Methods: This observational study was conducted using a web-based questionnaire. Patients who had received Treprost Inhalation for at least 30 days at the time of the survey response were included. Overall treatment satisfaction, changes in dyspnea symptoms, and activities of daily living were assessed. In addition, burden associated with inhalation procedures and the management of nebulizers and medications, as well as treatment implementation during outings, were analyzed.
Results: A total of 276 valid responses were included in the analysis. Overall, 59.1% of patients reported satisfaction with Treprost Inhalation therapy, and 53.3% perceived an improvement in dyspnea symptoms. No change in activities of daily living was reported by 59.4% of patients, representing the largest proportion of responses. Moreover, 91.7% of patients were able to independently perform inhalation procedures and manage nebulizers and medications, and 54.7% continued inhalation therapy even when outside the home.
Conclusion: Treprost Inhalation was associated with favorable overall treatment satisfaction and improvement in dyspnea. Furthermore, patient-reported outcomes indicated that many patients were able to independently perform inhalation procedures and manage nebulizers and medications.

Corticosteroid Use During Nipocalimab Treatment in Japanese Patients with Generalized Myasthenia Gravis: A Post-Hoc Analysis of the Phase 3 Vivacity-MG3 Open-Label Extension
目的:全身型重症筋無力症(gMG)患者では,症状コントロールを維持しながらコルチコステロイド(CS)使用量を低減することが重要な治療課題となっている。本解析では,日本人gMG患者におけるニポカリマブ治療下のCS使用状況を検討し,同患者におけるminimal symptom expression(MSE)の達成状況を探索的に評価した。
方法:本研究は,Vivacity-MG3試験のオープンラベル延長(open-label extension:OLE)期データを用いた事後解析である。二重盲検(double-blind:DB)期では,全ての症例がニポカリマブ群とプラセボ群にランダムに割り付けられ,24週間のDB期を完了してOLE期に移行した症例では,全例にニポカリマブが投与された。主要解析集団は,OLE期に移行した日本人患者の中で,OLE期ベースライン時点でCSを使用していた患者とした。OLE期ベースラインから最終評価時点(2024年8月データカットオフ時点;ニポカリマブ曝露期間中央値76週)までのプレドニゾロン換算CS 1日量の変化に加え,OLE期データカットオフ時点でプレドニゾロン換算5 mg/ 日以下を達成した患者割合,および24週,36週,48週までの間にMG-activities of dailyliving(ADL)総スコア0または1で定義したMSEの達成状況を評価した。
結果:日本人gMG患者11例がOLE期ベースラインでCSを使用していた。11例全体の平均CS 1日量は,OLE期ベースラインの9.2 mg/日からデータカットオフ時点には6.2 mg/日に低下した。7例(63.6%)でCSの減量が認められ,このうち5例(71.4%)が5 mg/ 日以下を達成した。CSの減量が認められた7例におけるMSE達成状況は,24週では0例,36週では3例(42.9%),48週では4例(57.1%)であった。
結論:Vivacity-MG3試験OLE期にCSを服用していた日本人集団において,ニポカリマブ治療下でCSの減量が一定割合で認められ,一部の患者ではプレドニゾロン換算5 mg/日以下およびMSEの達成が確認された。この結果は,CSを減量または中止し,症状が安定していた既報の全体集団の結果とおおむね一致しており,日本人においても,ニポカリマブはCSを減量しつつ,一部の患者でMSEを達成する可能性が示唆された。

Corticosteroid Use During Nipocalimab Treatment in Japanese Patients with Generalized Myasthenia Gravis: A Post-Hoc Analysis of the Phase 3 Vivacity-MG3 Open-Label Extension
Objective: In patients with generalized myasthenia gravis (gMG), reducing corticosteroid (CS) use while maintaining adequate symptom control remains an important therapeutic challenge. This analysis examined CS use in Japanese patients with gMG treated with nipocalimab and exploratorily evaluated the achievement of minimal symptom expression (MSE) in the same patients.
Methods: This was a post hoc analysis using data from the open-label extension (OLE) phase of the Vivacity-MG3 trial. In the double-blind (DB) phase, patients were randomly assigned to nipocalimab or placebo. Regardless of initial treatment assignment, all patients who completed the DB phase and entered the OLE phase received nipocalimab. The main analysis population comprised Japanese patients who entered the OLE phase and were receiving CS at OLE baseline. Change in daily prednisolone-equivalent CS dose from OLE baseline to the final assessment (August 2024 data cutoff; median nipocalimab exposure time of 76 weeks) was evaluated. The proportion of patients achieving a prednisolone-equivalent CS dose of ≤ 5 mg/day at the final assessment and achievement of MSE, defined as a Myasthenia Gravis Activities of Daily Living (MG-ADL) total score of 0 or 1, by Weeks 24, 36, and 48 were also assessed.
Results: Eleven Japanese patients were receiving CS at OLE baseline. In these patients, mean daily CS dose decreased from 9.2 mg/day at OLE baseline to 6.2 mg/day at the final assessment. CS dose reduction was observed in 7 patients (63.6%); of these, 5 (71.4%) achieved ≤ 5 mg/day. Among the 7 patients with CS dose reduction, MSE was achieved in 0 patients by Week 24, 3 patients (42.9%) by Week 36, and 4 patients (57.1%) by Week 48.
Conclusion: In the Japanese population who were receiving CS during the OLE phase of the Vivacity-MG3 trial, CS dose reduction was observed in a proportion of patients during nipocalimab treatment, and some patients achieved both a prednisolone-equivalent CS dose of ≤ 5 mg/day and MSE. These findings were generally consistent with previously reported results in the overall population, in which CSs were reduced or discontinued while symptoms remained stable. The results suggest that, in Japanese patients, nipocalimab may allow steroid dose reduction while enabling some patients to achieve MSE.

背景:The Semaglutide Effects on Cardiovascular Outcomes in People with Overweight or Obesity(SELECT)試験では,心血管疾患の既往があり,糖尿病の既往のない過体重※または肥満者(以降,肥満者とする)において,セマグルチドが主要心血管イベント(MACE)のリスクを低下させることが示された。現在,セマグルチドの心血管およびその他のベネフィットを検討するため,SELECT 試験の事前に規定された複数の解析が実施されている。
方法:SELECT試験において,セマグルチドの心血管およびその他のベネフィットを検討した事前規定の二次解析および探索的解析の報告を精査し,体重減少以外のセマグルチドのベネフィットについて検討した。
結果:心血管疾患の既往があり,糖尿病の既往のない肥満者において,セマグルチドは体重減少以外に心血管ベネフィットをもたらした。セマグルチドの有益な効果は,さまざまな心血管アウトカムおよびサブグループで一貫しており,多岐にわたった。これらの効果は体重減少の程度に依存せず,ベースライン時の年齢,性別,BMI,血糖状態(正常血糖または前糖尿病)および心不全サブタイプにかかわらず認められた。プラセボ群と比較して,セマグルチド群は腎アウトカムの改善,正常血糖への改善促進,糖尿病への進行抑制および臨床的に有意な体重減少と関連していた。さらに重要な点として,セマグルチド群ではプラセボ群と比べて,あらゆる原因による死亡率が低下し,感染症による死亡の減少により非心血管死亡率が大幅に低下したことが挙げられる。
結論:セマグルチドは体重減少だけでなく,心血管ベネフィットももたらすため,糖尿病の既往のない肥満者において,セマグルチドの治療で十分な体重減少が認められない場合にも有益である可能性が示された。また,セマグルチドはリスク集団におけるMACE発現率を低下させる治療選択肢となり得る。
※本試験に参加したBMI 27 kg/m2 以上30 kg/m2未満の患者を指す。世界保健機関(WHO)の基準ではBMI 25 kg/m2 以上30 kg/m2未満を過体重と定義する。日本肥満学会の基準では肥満に該当する。

Objective: To investigate whether a capsule containing Clostridium butyricum , 3-(4-hydroxy-3-methoxyphenyl) propionic acid (HMPA), and salmon milt-derived DNA improves cognitive function in middleaged and older adults with subjective memory complaints.
Method: This 12-week randomized, double-blind, placebo-controlled trial enrolled 80 adults aged 55-79 years with subjective memory complaints and MMSE-J ≥ 24. Participants received Test food or Placebo once daily. The primary outcome was cognitive function assessed by Cognitrax. Secondary/exploratory outcomes included bowel habits, biochemical parameters, and safety. Efficacy was evaluated in the per-protocol set (n=74), and safety was evaluated in the full analysis set (n=80). An exploratory subgroup comprised participants aged ≥ 65 years with baseline MMSE-J scores of 24-27.
Results: Overall, no consistent between-group differences were observed in memory-related Cognitrax outcomes, but Symbol Digit Coding correct responses improved significantly in the Test food group. In the exploratory subgroup, Verbal Memory and Verbal Memory delayed correct hits improved significantly, whereas Symbol Digit Coding errors decreased significantly, versus Placebo. Bowel habit indices did not improve. Exploratory analyses suggested favorable changes in fasting glucose, HbA1c, gamma-glutamyl transferase (γ-GT), and uric acid. No serious adverse events or product-related side effects occurred.
Conclusion: The Test food did not show consistent overall memory-related benefit, but improved an attentionrelated index and, in a cognitively high-risk subgroup, improved verbal memory-related outcomes while an attention-related subtest score decreased. Together with unchanged bowel indices and favorable exploratory changes in selected metabolic markers, these findings suggest that metabolic changes may be involved, at least in part, in the observed cognitive effects.
